HL7 - MSDS ASTM Joint Meeting Minutes (HL7 Orders and Clinical Data) Orlando, FL January, 1993 We reviewed the a very prelimary data model that is the work of the CEN TC251/PT008 working group committee. This is not an official release, but Georges DeMoor was kind enough to share this preliminary draft with Clem McDonald. The group began to flush out a data model focussing on the following categories: Observation Request Observation Subject Observation Report Sample Observation Definition Observation The following will not be discussed at this time: Image/Tracing Observation Provider Obseration Filler Report Provider A preliminary data model showing these items was approved. Favor: 13 Opposed: 1 Absention: 7 OBSERVATION Detailed discussion as follows. ID of Observation include in the model as an attribute forclarity SubID of Observation distinguishes duplicates Value requires a task force to define. No volunteers were found at the meeting. Data type of observ. the definition of Value must be determined prior to defining this element Units include in the model need for clinical observation Normal Ranges include in the model must be the units of the test above. Vote: Include: 20+ Do not include: 1 This element does require more research. Abnormal Flag include in the model needs a generic word to replace abnormal. The flag can vary aand should allow multiples. Nature of Ab. Testing How many can you have? Special confidentiality control include in the model optional access restriction Observation Date/Time time of collection, time took the pictures, time gathered history. There is no sample unless this is a specimen. When no sample, this is the date the information is acquired. When there is a derived sample, it is the date the specimen is acquired. This is the clinically significant time. [ There may need to be a physiological date/time element. This discussion is held until a future meeting.] ID of Observation Provider This is a link to the Observation Provider object. Responsible for Observation The person that verifies the observation is correct. ID of Sample Link to Sample object. Observation Status relates to Observation date/time For the future: If there is a need to support status history, then break these two elements out possibly to a Status object. Observation comments Probability skip for now, but keep as a place holder. The following fields were moved to Observation Definition: Equipment of test procedure Reagent of test procedure Temperature of test procedure Vote: Favor: 12 Opposed: 2 The key is the Observation ID + SubID + Observation Date/Time. OBSERVATION DEFINITIONS The following elements need clearer definitions: Preferred OBS ID Compound Principle of measurement Measurement procedure Method of measurement - List of? Effective Date/Time preferred to never change the use, but if change does occur due to lab accident, etc. use this. Date/Time Anything changed in procedure definition List of: cross reference to coding systems like ICD-9 Preferred name a medium size name Preferred short name mneumonic for observation List of: other names by which observations may be ordered Class of Observation the nature of it (batery, atomic observation, super set, calculated value, etc.) The rest of the Observation Definition element were skipped for a later meeting. The next meeting of this group will probably be held 1-2 days prior to the Boston HL7 meeting. It is recommended that all members become familiar with Coad and Yourdon approaches to object-oriented design prior to participation with the model development.
